• If you are paying with a credit card, but not ordering online, you can use this method!

• Fax this order form once you have downloaded, printed and completed the required information on it.
• Be sure to include a copy of your purchase order within your faxed order.

3. EMAIL:mail@mediaprojects.org

• When you download this order form to your computer, save it as a WORD file.
• Within the actual file, fill in the required information blanks on the form, and then save the form.
• Send this order form file within the body of your email or as an attachment to your email.
• You can order by email only by using a credit card. Indicate your credit card type, CC# and your card's Expiration Date in the form .

4. ONLINE: Click "Films" at upper left for complete list of films available for purchase.

• Click on the appropriate screen for the film you wish to order.
• If a PayPal button is available on that film's screen, you can purchase the film ONLINE using a credit card.
• Otherwise, note the film price for the appropriate purchasing category, return to this screen and download this order form so you can complete required purchase information and send the form to us.

5. MAIL: Media Projects, Inc.  5215 Homer St., Dallas, TX 75206

• Include a completed copy of your order form in your mailing.
• Be sure to include your payment!
• Make checks and purchase orders payable to Media Projects, Inc.

HERE'S HOW SHIPPING WORKS

1. Generally, we ship within 24 hours of receiving your order.

• We ship via UPS Ground Delivery for the continental 48 states. We have an excellent working relationship with UPS and will track your package on its entire journey to you!
• Most orders ARRIVE within 3 - 5 working days, depending on the distance the orders must travel from Dallas, Texas.
• If ordering during peak holiday periods, expect that additional days will be required for delivery.

2. If you are ordering from Alaska, Hawaii, or internationally.

• Additional shipping charges are necessary to ship orders to Alaska, Hawaii and internationally.
• Please contact us via phone or email, so that we can explain your shipment options/charges.
• We'll be happy to help you decide on the most cost-efficient method of delivery for your order.

3. Do you need EXPEDITED delivery?

• Should you require RUSH delivery of your order, please call us right away with your request.
• Additional shipping charges will apply.
• We prefer to take your RUSH order by phone to assure immediate filling of your order.
• We also clarify the upgraded shipping charges so you know your actual order total before we ship.
